In 1999, at a place called Glenbrook, just outside of Sydney, Australia, two trains collided killing seven people. In 2003, at a place called Waterfall, just outside of Sydney, Australia, a train derailed killing seven people. Same operator, same regulator, same state government, same judge leading the inquiry.
